Lash & Tint Consultation Form

These Terms and Conditions shall apply to all contracts for the supply of Services by the SUPPLIER to the Client and shall prevail over any other documentation or communication from the Client. Any variation to these Terms and Conditions shall be inapplicable unless agreed in writing by the Supplier.

PRICE AND PAYMENT 

​

• Payment of all appointments are made via In person, Online or Credit card. The booking system is not owned by the Supplier. It is a subscription service run by WIX .

• The SUPPLIER reserves the right to modify, update or run promotions on any service at any time.

The SUPPLIER reserves the right to change the price of any service at any time. Once a service has been ordered, the price shall remain fixed for the Client.

Under no circumstances shall the Tech refund the difference should the price of that service decrease.

LATENESS 

Please text if you are going to be late 5-­10mins late. Your treatment time will be shortened to accommodate your lateness, therefore, meaning  you  will only get the service that can be provided with your remaining appointment time. The Supplier will not extend your appointment time to accommodate your lateness. If you are  15 mins late, your appointment will be cancelled and your service fee will not be refunded. This is considered a NO SHOW.

LASH RETENTION

​

• The longevity of your lash extensions:UK LASH STUDIO does not guarantee how long your lashes will last. Retention lengths or aftercare notices that are in our website, printed materials, social media sites and what UK LASH STUDIO is only a guide. The longevity of your eyelash extensions are influenced by many factors including but not limited to; lifestyle, use of makeup products, cleansing routine, skin type, and lash growth cycle. 

• What do I do if my retention does not suit the number of weeks I've had my lashes? Please contact UK LASH STUDIO via Phone or email immediately to ensure that your next appointment is appropriately set. 

• Arriving at your appointment with low retention: Do not book or arrive at your appointment with retention lower than the compulsory base. If you do, you have the following options: Upgrade and pay to the appropriate touch-­‐up base or fullset; or Cancel your appointment and pay 80% of your booked service.

MEDIA

​

• The client understands and consents that media  i.e. photos, videos, audio etc. will be taken during their appointment. 

• The client agrees that all media i.e. photos, videos, audio etc. taken during my procedure or sent to UK LASH STUDIO are the property of  UK LASH STUDIO and can be used on all.  UK LASH STUDIO business-­related social media publications.

FEEDBACK

• Providing feedback: If you have feedback about our service, please do so via email We will contact you within 7 days for a resolution of your feedback.

• Social media feedback: If you access our service you shall not post, publish, upload or distribute any messages, data, information, text, graphics, links or other material ("Postings") on any social media platforms, email or electronic mediums about The Tech which is unlawful or abusive in any way, including but not limited to any postings that are defamatory, liabelous, pornographic, obscene, threatening, invasive of 

• privacy or publicity rights, inclusive of hate speech, or would constitute or encourage a criminal offense, violate the rights of any party, or give rise to liability or violate any local, state, federal or international law. You will be liable for any costs associated with any economic or social lose The Tech incurs due to post, publishing or upload.

WAIVER

• I agree that all appointments with The Tech [Have a deposit or are prepaid] via website and Booked in. 

• I understand there are risks associated with having artificial eyelashes applied to and/or removed from my natural eyelashes. 

• I understand that the eyelash extensions will be applied to the natural lash as determined by the technician so as not to create excessive weight on the natural eyelash thereby preserving the health, growth and natural look of the Client's natural eyelashes. 

• I understand as part of the procedure eye irritation, eye pain, eye itching, discomfort and in rare cases eye infection may occur. 

• I understand and agree that if I experience any of these issues with my lashes that I will contact my technician and have the eyelashes removed immediately and consult a physician at my own expense. 

• I understand that even though the technician may apply and remove the eyelashes properly, that adhesive material may become dislodged during or after the procedure, which may irritate my eyes or require further follow up care. 

• I understand and agree to follow the aftercare instructions provided by my technician. Failure to follow the aftercare instructions can cause the eyelash extensions to fall out. 

• I understand that in order to have the eyelash extensions applied to my eyelashes I will need to keep my eyes closed for the duration of my appointment duration during the procedure.

• I also understand that I will need to be lying in a reclined position. Any medical conditions that might be aggravated by lying still for a prolonged period of time may mean I will not be able to have the procedure performed on my eyes. I understand that this procedure requires single synthetic or mink eyelashes to be glued to my own natural eyelashes. 

• I understand that it is my responsibility to keep my eyes closed and be still during the entire procedure until my eyelash technically addresses me to open eyes.

• I understand that some risks of this procedure may be but not limited to eye redness and irritation. The fumes from the adhesive may cause eyes to tear up if I open my eyes. 

• I understand that I am required to follow the eyelash extension care sheet (detailed at www.uklashstudio.co.uk) to maintain the life of these extensions.

• I agree that by reading and submitting the consultation form, I release THE TECH from any claims or damages of any nature and i hold them harmless. 

• I also agree that once the procedure has commenced, I am not eligible for a refund of any sort. 

• I agree that rI have read and fully understand this entire consent form and I am of sound mind and fully capable of executing this waiver for myself. 

• I have read terms and conditions in its entirety, and have answered everything to the best ability. I have been informed of potentially harmful or negative side effects that may be caused by the application and/or removal of Eyelash Extension. 

• These terms and conditions are subject to change at any time. Please check www.uklashstudio.co.uk terms and conditions on a regular basis for updates. 

• I confirm and agree that I wish to engage the services of THE SUPPLIER to apply eyelash extensions.

• This agreement will remain in effect for all procedures conducted by THE SUPPLIER. I understand that this agreement is binding and that I have read and fully understand all the information listed above.

• I represent that I am over the age of 18 years. If below 18 years of age a parent or guardian must also sign this form

VALIDITY

​

•  If any covenant or obligations of the agreement or the application thereof to any person or circumstances shall be or become invalid or unenforceable, the remaining covenants and obligations shall not be affected thereby and each covenant and obligation of these Terms and Conditions shall be valid and enforceable to the fullest extent by law

ENTIRE AGREEMENT

• Subject to the terms of the agreement, these Standard Terms and Conditions contained within this document embody the entire understanding and agreement between the parties to this agreement as to the subject matter of these terms and Conditions.

• Subject to the terms of the agreement all previous negotiations, understandings, representations, warranties, memoranda or commitments in relation to, or in anyway affecting, the subject matter of these Terms and Conditions are merged in and superseded by these Terms and Conditions and shall be of no force or effect whatever 

• and no party to the agreement shall be liable to any other party to these Standard Terms and Conditions in respect of those matters. No oral explanation or information provided by any party to these Terms and Conditions to another shall affect the meaning or interpretation of these Terms and Conditions or constitute any collateral agreement, warranty or understanding between any of the parties to these Standard Terms and Conditions.

CHANGES TO OUR TERMS AND CONDITIONS 

• This policy is subject to change at any time and only the most current copy available applies and not the one you have photographed or stored. Please check (ADD PAGE INFO) Policy page on a regular basis for updates

• I have agreed to have eyelash extensions applied and/or removed from my eyelashes. Before my qualified professional eyelash technician can perform this procedure, I understand I must complete this agreement and provide my consent by submitting the consent form.
